MCP Learning Community Resources

The Medi-Cal Palliative Care Managed Care Plan (MCP) Learning Community was a project funded by the California Health Care Foundation. Below is a collection of useful tools and resources that were developed to assist managed care plans and providers working with patients who would benefit from receiving palliative care services. 

These tools were initially developed for the MCP Learning Community. However, many of them can support all health plan case managers and providers who work with seriously ill members and patients who would benefit from receiving palliative care, regardless of payer.

 

Introducing Palliative Care Video Series

This video series and supporting materials were developed to help case managers and clinicians become more comfortable with describing palliative care services or responding to common questions or concerns about palliative care when offering it to their members or patients.

Annual Surveys

Annual Plan and Provider Surveys were designed to capture unique differences between plans and providers, such as expanded eligibility criteria and percentage of overall palliative care business represented by Medi-Cal patients, as well as some of the same general categories and specific questions aimed at assessing the number of enrolled Medi-Cal participants, program and provider certification requirements, mechanisms for payment, caregiver supports, and sustainability. Responses were analyzed, and the data shared during the corresponding virtual convening.

Annual Convenings

An annual statewide Medi-Cal palliative care (virtual) convening was held in 2022 and 2023 as part of the Learning Community. The convenings provided attendees with an opportunity to reflect on the current state of Medi-Cal palliative care, share strategies and tools for assuring reliable delivery of high-quality palliative care, and consider how trends and advances in the field of serious illness care could impact Medi-Cal palliative care.
